The P3 Process of Care: Comprehensive Medication Therapy Management

The P3 Program™ Process of Care is an evidence-based therapeutic process delivered by trained pharmacists for the management of patients with chronic disease(s). This model prepares pharmacists for new roles in clinical care using the P3 process called Comprehensive Medication Therapy Management (CMTM) services. This interactive, patient-centered clinical service provides a framework for practitioners to improve the health status and self-efficacy of their patients. P3 Program™ CMTM services have been studied and proven to improve the clinical health outcomes of patients while reducing unnecessary costs of care. Learners will be introduced to the six-steps in the process with emphasis in the use of the Therapeutic Experiment to promote pharmacist-provided disease management.
